Please download and complete the Reasonable Adjustments form provided below and upload it along with supporting evidence (e.g., diagnosis report, doctor's letter) to the apprentice's profile in ACE360 in the shared documents area. Once that is done, select the blue "View Account Information" button and you will be taken to another screen where you can request Reasonable Adjustments by checking the tick box.
This will appear in the ACE360 dashboard and will prompt EPA Delivery to review your request. It usually takes 1-2 working days but, in some instances, may take up to 5 working days.

*Please note that for illnesses that have prevented an apprentice from undertaking their EPA and have resulted in going outside the three-month EPA window, Reasonable Adjustments are not applicable, for this type of scenario, please complete an Extenuating Circumstances form.
