End-point Assessment: Missing EPA Certificate

Apprentice has achieved their EPA but has not received their certificate, how do we track the EPA certificate?

Please provide the EPA Delivery team with the following details:

• Apprentice name(s)
• ULN (unique learner number)
• Standard
• Employer Contact
• Employer Address or Certificate Address

The above information can be found within the apprentice record within ACE360, once obtained, the EPA Delivery team can contact the Department for Education to track the whereabouts of the EPA certificate.
